The 15 bed transitional care unit is reopening at the Bayonne Medical Center following approval from the New Jersey Department of Health, CarePoint Health announced today.

This TCU facility, equipped with private and semi-private rooms, will operate 24 hours a day, seven days a week, after the service was suspended during the COVID-19 pandemic.
“Our commitment to the health and well-being of our community has never wavered, even during the darkest days of the pandemic when conditions forced programs like the Transitional Care Unit to shut down,” CarePoint President and CEO Achintya Moulick said in a statement.
“The re-opening of the Transitional Care Unit at Bayonne Medical Center is a testament to our dedication to providing exceptional health care services to Hudson County residents. This facility will help empower our patients to regain their health and independence while they make the adjustment from hospital care to returning home.”
The BMC Transitional Care Unit will serve as a vital resource for individuals who meet specific admission criteria, including patients who have had a minimum of three overnight stays in an acute care hospital based on approved diagnoses, patients requiring skilled nursing and/or rehabilitation services five days per week, and patients anticipating meeting nursing and rehabilitation needs for eight days or longer.
Additionally, patients “with an acceptable admitting diagnosis documented in their medical record” and who are not current residents of a long-term care facility will be accepted.

Bayonne Medical Center, with its 278-bed capacity and full accreditation, has been serving the community for over a century.
